Joe Koloc
Physical Therapist (PT), President

Joe is a lifetime resident of Lake Wales. He began his career in physical therapy after finishing his tenure in the US Army.

He has degrees in Physical Therapy from the University of Central Florida, and an MBA from Webber International University. After working for 18 years in various settings including local hospitals, the VA, and outpatient clinics, he opened Core Rehabilitation in 2013.

Joe is proud of his ability to bring 25 years of clinical experience and is a back-pain and shoulder specialist. He is a member of the American Physical Therapy Association and serves on Keiser University’s Lakeland Campus Physical Therapy Assistant Clinical Review Committee.

Leonilo β€œLeo” Trono
Physical Therapist (PT), Massage Therapist

Leo was born in Butte, Montana and raised in Kissimmee, Florida. He received his massage therapy diploma from Everest University in Orlando, Florida. He worked for several years as a medical massage therapist before pursuing his physical therapy degree. He received his physical therapy degree from The University of Perpetual Help in Las Pinas City, Philippines. Leo is a proud father; he believes he was destined to be a great father and a great therapist. He aspires to finish his career in an exercise laboratory developing new techniques which will be taught to future physical therapists.

Valerie Smith
Physical Therapist (PT)

Valerie is a native of Canada. She is our PT in the Haines City office. Valerie has a Masters of Physical Therapy from Laval University. She is passionate and enthusiastic about her work. She strives to create a clinical environment that helps each and every one of her patients achieve their full potential. In her spare time Valerie enjoys cooking, traveling, and is an avid marathon runner!

Jason Hammett
Physical Therapist Assistant (PTA)

Jason is a native of Lakeland. He has a Physical Therapists Assistant (PTA) degree from Keiser University. He served 8 years in the US Marine Corp, and 3.5 years in Marine Reserves. He enjoys the outdoors in his spare time.

Raymond Hernandez Chacon
Physical Therapist Assistant (PTA)

Ray has over 15 years of experience in the physical therapy world. He graduated from Polk State College in 2013 and has spent the last several years as a licensed physical therapist assistant. In his free time, Ray enjoys time with family and going to the movies. He is a huge fan of Star Wars and the Buccaneers.

Justin Burgos
Physical Therapist Assistant (PTA)

Justin is a PTA working in both our Lake Wales and Haines City clinic? Justin graduated from Keiser University in 2019 as a Physical Therapist Assistant. He is a native of Oregon, but was raised in Texas. He and his wife relocated to Florida about 2 years ago. Justin loves to fish, play sports, and lift weights.

Diana Koloc
Operations Manager, Vice President

Diana is a lifetime resident of Lake Wales. Prior to Core Rehabilitation, Diana opened, and operated two businesses in the health and beauty industry. Diana attended both Polk State College as well as Warner University earning an Associate degree in Art as well as a Bachelor’s degree in Organizational Management. Diana volunteers at a local hospital, political rallies, is a table leader for a Bible Study for Women, and a member of the support group SOBBS. Diana does a lot of behind the scenes work at Core Rehabilitation and we are thankful for all that she contributes to make Core Rehabilitation run smoothly.

Savannah Russell
Patient Administration Specialist

Savannah has been with Core since 2017 and currently serves as the PAS in our Haines City clinic. She is a super important part of our team!

She is a native of Polk County. In her free time, Savannah enjoys spending time with family and friends, and crafting.

Anjelica Cruz
Patient Administration Specialist

Anjelica is from the small-town Avon Park. She graduated from South Florida State College with a Certification in Medical Administrative Specialist with distinction and a 4.0 GPA. She is proud of her Mexican-American heritage and can also speak Spanish and English fluently as well as reading and writing.

Laura Aguirre
Patient Administration Specialist

Laura is the new PAS in our Haines City clinic. She is fluent in English and Spanish. Laura is the youth choir director at her church and has a passion for helping all in need. Laura is part of the alumni at Southeastern University, she is currently studying Nutrition and herbalism.
