Joe Koloc
Physical Therapist (PT), President

Joe is a lifetime resident of Lake Wales. He began his career in physical therapy after finishing his tenure in the US Army.

He has degrees in Physical Therapy from the University of Central Florida, and an MBA from Webber International University. After working for 18 years in various settings including local hospitals, the VA, and outpatient clinics, he opened Core Rehabilitation in 2013.

Joe is proud of his ability to bring 25 years of clinical experience and is a back-pain and shoulder specialist. He is a member of the American Physical Therapy Association and serves on Keiser University's Lakeland Campus Physical Therapy Assistant Clinical Review Committee.

Jason Hammett
Physical Therapist Assistant (PTA)

Jason is a native of Lakeland. He has a Physical Therapists Assistant (PTA) degree from Keiser University. He served 8 years in the US Marine Corp, and 3.5 years in Marine Reserves. He enjoys the outdoors in his spare time.

Raymond Hernandez Chacon
Physical Therapist Assistant (PTA)

Ray has over 15 years of experience in the physical therapy world. He graduated from Polk State College in 2013 and has spent the last several years as a licensed physical therapist assistant. In his free time, Ray enjoys time with family and going to the movies. He is a huge fan of Star Wars and the Buccaneers.

Dave Dangan
Physical Therapist (PT)

Dave graduated from Our Lady of Fatima University, Philippines as Physical Therapist. He has a background in volunteer work with hospital systems and received his license in 2018. Dave moved from Michigan after working in assistive care in 2020. Dave joined Core Rehabilitation in 2020 and enjoys working in the outpatient setting and specializing in back pain. He especially enjoys working within geriatrics because he finds it rewarding to see his patients get better and hearing real life stories and life lessons from them.

Diana Koloc
Operations Manager, Vice President

Diana is a lifetime resident of Lake Wales. Prior to Core Rehabilitation, Diana opened, and operated two businesses in the health and beauty industry. Diana attended both Polk State College as well as Warner University earning an Associate degree in Art as well as a Bachelor's degree in Organizational Management. Diana volunteers at a local hospital, political rallies, is a table leader for a Bible Study for Women, and a member of the support group SOBBS. Diana does a lot of behind the scenes work at Core Rehabilitation and we are thankful for all that she contributes to make Core Rehabilitation run smoothly.

Savannah Russell
Claims Specialist

Savannah is a native of Polk County and has been with Core since May 2017. Savannah started off as our Haines City Patient Administrative Specialist and now serves as our Claims Specialist. She is a super important part of our team and can help with any insurance questions you may have.

Johana Perez
Patient Administration Specialist

Johana is our Patient Administrative Specialists and receptionist in Lake Wales. She is originally from Texas and moved to Florida six years ago. She has a background in working with and training in the Red Cross for providing first aid which she studied in Mexico. She credits her quick computer skills to her enjoyment of video games and she is a critical member of our Lake Wales office front desk team. She is ready to help you with any of your insurance or scheduling issues.

Maria Mateo
Patient Administration Specialist

Maria is our Patient Administrative Specialist and receptionist in Haines City. She was born in the Dominican Republic and grew up in New Jersey. She volunteers for causes that she is passionate about. She speaks 3 languages and hopes to become an even more accomplished linguist by learning 2 additional languages. She has a wide range of hobbies and skills and hopes to bring all of her experience to bear while working as our Haines City Patient Administrative Specialist.

Alisha Lopez
Patient Administration Specialist

Alisha is our Patient Administrative Specialists and receptionist in Lake Wales. She is a graduate of Florida Technical College with a certification as a medical assistant. Alisha is originally from the Tampa area; she is interested in furthering her career in holistic medicine and basis her life on the principles of her faith.
