Welcome to Core Rehabilitation! We are a private practice provider of physical therapy services in Lake Wales and Haines City, Florida. We strive to be the leading clinic offering comprehensive physical therapy in Central Florida.
Established in 2013, Core Rehabilitation offers the most up-to-date treatments based on the latest evidence based scientific approach available. We specialize in the caring and rehabilitation of difficulty back pain and shoulder pain.

Joe Koloc | Physical Therapist MSPT, MBA, and President
Joe Koloc | Physical Therapist MSPT, MBA, and President
Joe is a lifetime resident of Lake Wales. He began his career in physical therapy after finishing his tenure in the US Army. He has a degree in physical therapy from the University of Central Florida and an MBA from Webber International University. After working for 18 years in various settings including local hospitals, the Veterans Affairs, and outpatient clinics, he opened Core Rehabilitation in 2013.
Joe is proud to bring more than 25 years of clinical experience in neck and shoulder pain. He is a member of the American Physical Therapy Association and serves on the Keiser University's Lakeland Campus Physical Therapy Assistant Clinical Review Committee.

Dave Dangan | Physical Therapist
Dave graduated from Our Lady of Fatima University, Philippines as physical therapist. He has a background in volunteer work with hospital systems and received his license in 2018. Dave moved from Michigan after working in assistive care in 2020.
Dave joined Core Rehabilitation in 2020 and enjoys working in the outpatient setting. He specializes in back pain and prefers working within geriatrics because he finds it rewarding to see his patients get better and hearing real life stories and lessons from them.

Diana Koloc | Operations Manager, Vice President
Diana is a lifetime resident of Lake Wales. Prior to Core Rehabilitation, Diana opened and operated two businesses in the health and beauty industry. Diana attended both Polk State College as well as Warner University, earning an Associate degree in Art as well as a Bachelor's degree in Organizational Management.
Diana volunteers at a local hospital and in political rallies. She is also a table leader in women’s Bible study and is a member of the support group SOBBS. Diana does a lot of behind-the-scenes work at Core Rehabilitation and we are thankful for all that she contributes to make Core Rehabilitation run smoothly.
Avelino Lopez | Physical Therapist Assistant
Avelino Lopez | Physical Therapist Assistant
Avelino was born and raised in Florida. He attended Polk State College and earned his degree as a physical therapy assistant in 2020. Avelino is close to his family and friends. He enjoys spending time with them and working out in the gym.
He is proud of his work as a PTA and finds that helping people has made it a very rewarding career. He specializes in treating the back with manual therapy and exercises. He seeks new opportunities to help his patients with new treatment approaches.
Vincent L. Santiago | Patient Administrative Specialist
Vincent was born in Coral Springs, FL but grew up in Cabo Rojo, Puerto Rico. He served in the US Navy for 7 years. He is new to the Physical Therapy community and continues to learn and the hard work deliver excellent customer service and treat everyone with outstanding kindness as part of the Core Rehabilitation Team!
Vincent loves cooking, art, and enjoys listening to a wide variety of music. He also has two pet ferrets named Quasimodo and Giuseppa.
Lauren Hudson | Physical Therapy Assistant
Lauren graduated from the Polk State Colleges PTA program and joined Core Rehabilitation in January 2022. She moved from Texas to attend this PTA program and has since settled down in Winter Haven, Florida. Lauren enjoys spending time with her family, especially her grandmother whenever she can. She loves going to Disney World, as well as bonding with her cat Gypsy and her dog Finley.
Caitlin Hammock | Doctor of Physical Therapy, DPT
Caitlin was raised in Miami, Florida where she completed her Bachelors in Health Sciences from the University of Central Florida. Caitlin graduated from Arcadia University in Glenside, Pennsylvania with a Doctorate in Physical Therapy. At a young age, Caitlin started working with individuals with special
needs in the areas of sports recreation, academics, social and living skills. Caitlin continues to volunteer with the Special Olympics in the Central Florida area. Caitlin has always been passionate about sports and is an avid Miami Dolphins fan, having traveled to see them play in different cities. Caitlin has
recently joined Core Rehabilitation in 2023 and enjoys working in the area of orthopedic and sports rehabilitation. She proudly works with everyone who comes into the clinic and is always pleased to hear about everyone’s journey and how she can assist them in having a fulfilling life free of any discomfort.
Gabriel Koloc | Insurance Benefits Coordinator
Gabriel Koloc | Insurance Benefits Coordinator
Gabriel is an integral part in ensuring that all patients are pre-qualified and authorized by their insurance to receive care. He is critical in assisting patients in understanding their financial responsibilities for copays, co-insurances, and deductibles. He is also critical in assisting both the administrative and clinical staff in their jobs.
Gabriel is working on his 4-year degree with components in business, humanities, and American literature. He enjoys writing and is in fact working on a non-fiction thriller/mystery novel in his free time. Gabriel also loves music, coffee, and his sweet dog named Melania.
Leila Vazquez | Patient Administrative Specialist
Leila is the newest member of our team as a Patient Administrative Specialist in the Haines City office. She was born in Chicago, raised in Puerto Rico and been living in Florida for 1 year. Prior to Core Rehabilitation, she was working in Atlanta, GA as a infants/toddler teacher.
Seasons Avila | Patient Administrative Specialist
Seasons is a Polk native that graduated the Medical Administrative Specialist program at Ridge Career Center in 2007. She's spent time as a home health aide, a customer service rep and has now joined the core team as the Winter Haven office receptionist. She is fully bilingual and has lived all over but always
comes back to Polk. She spends her free time with her family, doing genealogical research and is active in her parish.
Rafi Lara | Physical Therapy Assistant
Rafael was born and raised in Trujillo Alto, Puerto Rico. He is a former college baseball player at Northwestern Oklahoma State where he earned his Bachelor in Health Sports Science/ He later attended Keiser University earning his degree as a Physical Therapist Assistant in 2024.
Rafael is proud to work with everyone who comes into the clinic and believes laughter is a great medicine. Fun fact about Rafael, he is a big Tampa Bay Rays Fan.
